Understanding Giddiness vs. Vertigo

While often used interchangeably, giddiness typically feels like lightheadedness, unsteadiness, or a faint sensation, whereas vertigo is a distinct rotational illusion—a feeling that you or your surroundings are spinning.

These symptoms arise when there is a mismatch between the signals sent by the inner ear vestibular system, visual pathways, and the central nervous system (brainstem and cerebellum). An accurate clinical assessment is essential to distinguish benign peripheral vestibular causes from potentially serious central neurological conditions.

Key Conditions We Evaluate & Manage

BPPV (Positional Vertigo)

Brief, intense spinning episodes triggered by changes in head position are effectively managed through canalith repositioning maneuvers.

Vestibular Migraine

Dizziness or vertigo is associated with migraine mechanisms, and may or may not occur alongside traditional headache symptoms.

Central Vertigo & Cerebellar Issues

Balance disturbances resulting from brainstem or cerebellar stroke, multiple sclerosis, or vascular compromise requiring urgent neurological care.

Vestibular Neuritis & Labyrinthitis

Acute inflammatory condition of the vestibular nerve causing sudden, prolonged severe vertigo, unsteadiness, and nausea.

Diagnostic & Therapeutic Approach

  • Bedside Neuro-Otological Exam: Detailed evaluation of eye movements (nystagmus), balance (Romberg, tandem gait), and position tests (Dix-Hallpike).
  • Neuroimaging: Protocol-driven brain MRI/MRA to rule out posterior circulation strokes, vascular malformations, or acoustic neuromas when indicated.
  • Repositioning Maneuvers: In-clinic procedural maneuvers (Epley, Semont) for swift resolution of benign positional vertigo.
  • Vestibular Rehabilitation Therapy (VRT): Specialized exercise regimens to promote central brain compensation for persistent balance deficits.

Warning Signs Requiring Emergency Attention

Seek immediate emergency medical care if vertigo is accompanied by sudden weakness or numbness on one side of the face/body, slurred speech, double vision, difficulty walking, or a severe, sudden-onset headache.
